The Veterans Service Office has the wellness forms that need to be filled out prior to going to Walgreens. Veterans can pick up this form at the old courthouse located at 1 West Hill Street in Wabash, or call the Wabash County Veterans Service Officer, Samuel Daugherty at 260-563-0661 extension 1260.
