WHAT ILLNESS OR DISABILITIES?  —She has a broken hip and kidney disease.

HOBBIES OR PASTIMES —Carolyn enjoys sewing, crocheting, and working with her hands.

DIET RESTRICTIONS:  diabetic.

COMMENTS/FAMILY —Carolyn has four children.  Her husband passed in February and he was a minister for 45 years.  They lived in Evansville, but she wanted to be closer to relatives.

OCCUPATION —Carolyn taught pre-school for five years.
