Hazel E. Hawkins, 86, Jonesboro, died at 5:45 p.m. on Thursday, June 16, 2016, at Twin City Health Care, Gas City. She was born in New Boston, Ohio, to the late Hamer and Ollie O. (Davis) Shiveley. On October 3, 1981, Hazel was married to Edward L. Hawkins, and he preceded her in death.

Hazel was a graduate of Epworth High School and received a Medical Assistant degree from IU Kokomo. She worked at UTEC in Huntington and previously at Owens-Illinois. She held her church membership at Apostolic Friends Church and, for the past several years, attended Westview Wesleyan Church. Hazel was a former member of Gas City Moose Lodge, where she was also past Chaplain and Sentinal. She was also active with Loyal War Mothers in Summitville.

One of Hazel’s favorite hobbies was photography. She also like working in her garden. She will be best remembered for how she loved to talk and how much she enjoyed spending time with her grandchildren.

Survivors include her son, John Lloyd; daughter, Janet Mau, Herscher, Illinois; grandchildren, Carrie Lloyd, Jonesboro, and Avery (AJ), Zachary, and Sophia Mau, Herscher, Illinois; great-grandchildren, Katie and Danielle Thompson; great-great-grandson, Jacob Butcher; brother, Rev. Richard (Palma) Shiveley; and sister, Margaret Horn.

In addition to her parents and husband, Hazel was preceded in death by a son, Samuel Lloyd; son-in-law, Robert Mau; brothers, Everett (Marie) Shiveley and Lester Shiveley, Sr.; and sister, Lucille Bollhoefer.

Family and friends may call from 12-1 p.m. on Wednesday, June 22, 2016, at Needham-Storey-Wampner Funeral Service, Storey Chapel, 400 E. Main St., Gas City, IN. Services will begin at 1 p.m. with Pastor Mick Simpkins officiating. Burial will follow at Gardens of Memory, Marion.
